Thank you for your interest in applying for the Youth Peer Advocate Provisional Credential.  We hope you share our excitement about this very important milestone in the history of Youth Peer Support in New York State!   

Before you begin, please read the Information for YPA Credential Applicants FAQ.  This will help you determine if you are eligible to apply and give you a better understanding of the requirements and the process.

Attention: New Application Criteria! 

Provisional (temporary) Credential applications will only be processed from candidates currently employed in NYSOMH licensed, designated or funded programs providing Medicaid reimbursable services of Youth Peer Support or other state approved programs.

NOTE: Applications MUST be completed IN FULL to be eligible for review

As you navigate through this application you will be asked to provide the following: 

• Employment Verification Form

• Proof of age  

• Proof of education 

• Level 1 YPA Training certificate 

• Two (2) letters of professional skills recommendation 

• Lived Experience statement 

• Code of Ethics attestation
