Information for FPA Credential Applicants FAQ 

As you navigate through this application you will be asked to provide the following: 

⦁     Proof of age

⦁      Proof of education 

⦁      Level 1 FPA Training certificate

⦁      Two (2) letters of professional skills recommendation

⦁      Lived experience statement 

⦁      Code of Ethics attestation
