PROVISIONAL Credential 
Application




Thank you for your interest in applying for the Family Peer Advocate Provisional Credential.  We hope you share our excitement about this very important milestone in the history of Family Peer Support in New York State!  
 
Before you begin, please read the Information for FPA Credential Applicants FAQ.  This will help you determine if you are eligible to apply and give you a better understanding of the requirements and the process.

You will need to download, fill out, and attach the following documents for your application:
⦁      Comparable Skills Form (only if you do not have a high school degree or higher level of education.)
⦁      Professional Skills Recommendation
⦁      Code of Ethics with signature page

You will also need to provide the following documents in your application:
 
⦁     Level One PEP Training Certificate
⦁     Proof of age
⦁     Proof of Educational Achievement
⦁     Two letters of reference

⦁     Statement of Lived Experience
